Calendars and Attendance Sheets
Please review the school calendar for information on important dates and events throughout the academic year. The Fall and Spring attendance sheets are to be utilized over the course of field placement to log the weekly dates and field work hours. A copy of attendance sheets are submitted at the end of each semester to the Field Education Department.
Six Week Assignment Forms
The six week assignment form is completed by the student and field instructor at approximately six weeks into the field placement. This form is to be submitted to the field education office in Garden City or to the off-campus centers.
Evaluation Forms
Evaluations are completed at the end of the fall semester (mid-year evaluations) and the spring semester (final evaluations). Evaluations for M.S.W. students in extended assignments (14 hours per week) are due at the end of February (mid-year) and in the beginning of August (final). These forms are completed by the field instructor and submitted to the Field Education Department. An evaluation cover sheet, the original evaluation and one copy are required. Instructions are provided with the evaluation mailing sent to all field instructors.

Field Manual

This form is to be completed by an agency that would like to establish an affiliation with the School of Social Work Field Education Program.

This form is to be completed by prospective or first time field instructors.
Requests for Students
This form is to be completed annually by field agencies who are seeking students for the upcoming year. Information about the number and level of students requested for placement, nature of assignments, agency hours of operation, and field instructor information is included. The form can be completed online or downloaded and sent to the Garden City Campus Field Education Office.
Work Study
Students who are currently employed in social service agencies may apply for work study field placement within their employment setting. Work/study applicants must meet specific criteria outlined in the Work/Study Guidelines and submit an application signed by the student, proposed field instructor and agency director. The application should also include a Field Instruction Agency Application Form and Field Faculty Personnel Form which are both completed by the agency director and proposed field instructor.
The completed work/study application and supporting documents should be submitted to the Garden City Field Education Office or off-campus Center
Employment References
Faculty field liaisons prepare an employment reference for graduating students. This reference is the formal statement by the School of Social Work summarizing the student’s educational experience, performance in class and field, and attainment of practice competencies at the time of graduation.
Faculty Recommendation for Field Placement
The Faculty Field Liaison completes this form for each continuing B.S.W. and M.S.W. student planning for their next year field placement. It describes the current year’s field assignments, student's overall field performance, and specific recommendations for the senior or second-year field work placement.
